The Gardens at Wyoming Valley

Mary came to us on February 19, 2019, she is a 64 year old female who was admitted to our facility following a hospitalization for lower extremity cellulitis, UTI, renal and respiratory failure. Prior to hospitalization, patient was independent with all functional tasks. She resided home with grandson.

Upon evaluation Mary was unable to stand and ambulate. She required total assistance for bathing, dressing, toileting, grooming and functional transfers. Mary presented with decreased activity tolerance.

Mary participated in physical, occupational and speech therapy services. She was cooperative and motivated toward her therapy goals. She received ADL training, gait training and cognitive training. Mary progressed toward her goals of independent. Upon discharge of therapy services, Mary reached an independent level for all aspects of care. Mary was discharged to an apartment at an independent level.
